Our clients based in South Dublin are seeking a Client Care Manager to perform a variety of duties in Client Care services in the Community.

The CCM will ensure the Company provides “best practice” quality care to our Clients by managing and developing the relationships with key stakeholders in the community.

You will ensure that the highest quality of care is delivered to our Clients. You will be capable of managing all aspects of the day-to-day Client Care operations, take responsibility and accountability and possess excellent communication skills.

CORE RESPONSIBITIES (Summary):

  • Develop and maintain positive relationships with all Clients through Quality Assurance (QA) visits and phone calls
  • Conduct QA visits with Clients in person or over the phone according to the QA frequency guidelines – conduct follow up as necessary.
  • Introduce new CARE Givers to Clients at commencement of services
  • Log all QA visits and phone calls in the Client file, on People Planner (internal system) & file appropriate report sheets in the relevant Client file.
  • Conduct safety checks during the Client home visit – Home Safety Risk Assessments and Manual Handling Assessments.
  • Follow up with any cause of concerns from CARE Giver and family, follow up with any changes in Client, cognitive, decline in the Client mobility, skin integrity, report liaison with the intervention team PHN, OT, Physio.

Required Criteria: 

  • A minimum of 2 years experience in Homecare
  • Full QQI Level 5 in Healthcare and QQI Level 6 Supervisory Management. (or working towards)
  • Have strong, communication, negotiation and relationship skills.
  • Excellent IT skills
  • Flexibility to work outside normal working hours if necessary
  • Driver with full licence
